To request for appointment and online booking system
  2. check box for “This request is not an emergency or urgent matter. I wish to proceed”

  3. fill out form, make sure information is accurate
  4. click “next”
  5. Fill out information for luminello account or signin at the bottom of the screen if you already have an account.
  6. Click “submit request” and we will process your request as soon as possible.
  7. Once you have received confirmation on your request, you can login into luminello to schedule your appointment.
