If you are new patient, please make at least 60 minute appointment (preferably via face-to-face) for an initial intake. New patient also please choose either child or adult intake forms and bring the filled form to your appointment. Please also sign telepsychiatry content form if you wish to be seen via telepsychiatry in the future.
